4/4/2024 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, Client was calm and attentive during the session. She shared that today was a rough day because she kept getting into trouble and arguing with her mother about her being distant and not showing any interest in the family gatherings. Behavior: Client shared that she tries to stay calm but it’s hard because she gets really frustrated with everyone easily. Client expressed that sometimes she feels her mother is picking with her over small things and she is starting to get frustrated. Client became irritated as evidenced by taking deep breaths and sucking her teeth. Intervention: QMHP-C helped Client develop healthy self-esteem by reducing level of anxiety and emotional reactions related to school and other independent functioning which have been noticed to be affecting Client at home and in the community. QMHP-C praised Client for being on task and following directions during the session. QMHP-C supported Client making good decisions and having positive behaviors. Intervention: QMHP-C talked to Client about her confidence levels and how they can affect her thoughts and behaviors. QMHP-C encouraged Client to utilize positive self-talk as a means of decreasing anxiety and managing frustration when encountering difficult or challenging circumstances. QMHP-C reminded Client of her strengths and abilities. QMHP-C praised Client for her efforts and being respectful to herself and everyone around her. Client demonstrated she can be a leader and a positive role model. Response: As Client developed understanding of expectations, she reported increased satisfaction and function. Client responded well to feedback, interacting well with others. Client was able to speak calmly and demonstrate her improved communication skills while engaging in self-reflection. Client implemented her plan for restoring relationships, and this was reviewed. Client smiled as she saw QMHP-C's positive gestures. Response: Client was able to process making good choices with QMHP-C and her mom. Client listed 3-4 behaviors that she was going to utilize in efforts to remain positive behaviors. Client appeared to be receptive to receiving positive reinforcement from mom and counselor as demonstrated by her smiling and open body language. The frequency and intensity of Client's defiant affect decreased. Client continued to work on the psycho social activity. Client engaged in self-reflection, discussing good and bad parts of her day.
